Patients who need an organ transplant can refer themselves or be referred to Clarian Transplant by their physician. Call 800-382-4602 or transplantinfo@clarian.org to begin the qualification process for acceptance into our program. A transplant coordinator and social worker will walk you through the entire process.
Our staff will not only walk you through the qualification process, but also will help you and your family with other needs, such as insurance and housing.
